An Introduction to Your Dosage of Fragmin
The dose of
Fragmin® (
dalteparin sodium) your healthcare provider recommends will vary, depending on a number of factors, including:
- The reason you are using Fragmin
- Your weight
- Your kidney function
- Other medical conditions you may have.
As is always the case, do not adjust your dosage unless your healthcare provider specifically instructs you to do so.
Fragmin Dosing for Preventing Deep Vein Thrombosis (DVT) Clots
The standard dose of Fragmin for preventing blood clots in people undergoing abdominal (stomach) surgery is 2500 IU once daily for 5 to 10 days, starting 1 to 2 hours before surgery.
For especially high-risk people, a higher dose of 5000 IU once daily for 5 to 10 days, starting the evening before surgery, may be used. An alternative regimen for especially high-risk people is 2500 IU, starting 1 to 2 hours before surgery, 2500 IU 12 hours later, and then 5000 IU once daily for a total of 5 to 10 days of treatment.
There are three different standard dosage regimens for Fragmin for preventing blood clots in people undergoing
hip replacement surgery, as follows:
- Fragmin 2500 IU 4 to 8 hours after surgery, then 5000 IU once daily
- Fragmin 2500 IU within 2 hours before surgery, 2500 IU 4 to 8 hours after surgery, and then 5000 IU once daily
- Fragmin 5000 IU 10 to 14 hours before surgery, 5000 IU 4 to 8 hours after surgery, and then 5000 IU once daily.
For all three
hip replacement regimens, 5 to 10 days of total treatment is standard.
The standard dose for preventing blood clots in people who are acutely ill (and are immobilized) is Fragmin 5000 IU once daily.
